Appointments must be made at least 2 days in advance. Please allow 24–36 hours for a response to your appointment request. If you are not notified within 2 working days of the submission of this form please call us at (623) 849–0700.

Appointments may be made Monday–Friday 9am–11am and 1pm–5pm.
NEW! Now also Saturday appoinments available between 8:30am and 12pm.
If you want to set up an appointment for a routine surgical procedure or dental cleaning, please call the hospital for current surgery days.
